Survey privacy notice.
Updated October 14, 2026 · Notice version v5
This voluntary survey helps ROTHAL LAB, based in British Columbia, Canada, understand facial shine and guide commercial product development. It is for Canadian adults aged 18 and older. Participation does not subscribe you to emails.
Your answers
We ask choice questions and optional written questions about your experience, current approaches, buying interest and price expectations. We do not ask for your name or email address. Please leave out names, contact details and medical information. Written answers or combinations of details could identify someone, so we do not promise that responses or the entire visit are completely anonymous.
We store submitted answers, the submission time, a response ID, the privacy-notice version for new responses, and whether a response came from Google Forms or this survey. New response IDs are randomly generated; imported IDs identify legacy records. The application does not link them to its separate abuse-control records. Responses support research and product development. Individual responses are not publicly displayed; the owner can review and export them through restricted access. Research support may receive relevant information when needed, with identifying details removed where possible.
Technical information and hosting
The survey database does not store raw IP addresses. Submission limits use a daily changing pseudonymous value derived from the connection IP, stored separately from answers. Previous days’ limit records are cleared when a later submission is processed. We do not add advertising pixels, third-party analytics or cookies to link visitors with their answers.
Our OpenAI Sites and Cloudflare hosting providers receive connection information and may process IP addresses, device information and logs. A Cloudflare security cookie is used; owner sign-in uses authentication cookies. We have not independently confirmed every provider log or backup retention setting, and do not promise an anonymous visit or storage only in Canada. Information may be processed outside Canada. Hosted data is covered by the Sites Data Processing Addendum.
Your choices and retention
You may skip optional questions or leave before submitting. By submitting, you choose to provide your answers for this research. We seek additional consent where required for a different use of personal information. Unsent answers are held in page memory and may be cleared by leaving or reloading. Keep the response reference shown after submission if you may contact us about your answers.
We review individual responses at least every six months and aim to delete or irreversibly aggregate them within 24 months, or earlier when no longer needed. A documented legal or continuing business need may justify longer retention with review. Removal is handled by the owner, not automatic expiry. A minimal action log supports accountability and prevents removed records being reimported. Earlier Google copies, exports and provider backups require separate handling; live-database deletion does not instantly erase every copy.
Privacy contact
For access to or correction of personal information covered by applicable law, withdrawal of consent, deletion requests or complaints, contact Privacy Officer, ROTHAL LAB: hello@rothallab.com. Provide your response reference, approximate date and enough context to locate the record. We may need minimal verification; a reference alone is not proof of identity. We can retrieve and remove a verified response when it can be safely located, subject to lawful retention requirements. Deletion is not an unconditional right to erase every copy. If a safe match is impossible, we will explain and offer assistance. We respond within applicable legal periods and explain any permitted extension or refusal.
